What problems were you having that caused you to seek treatment at our office?  Aches & pains all over, especially my right arm from fingertips to shoulder.

Your problems are a result of what?   I had whiplash 2 times and working at a desk for 20 years was no help

How long have you had these problems? Several years

Please state how these health problems have interfered with your normal life’s activities and relationships?  Can’t stand for long periods of time, can’t use right hand some times. 

How did you find out about our office?   Looking for someone close to home, visited Dr. Allred and finally came back.

When did you start care at our office?    This time January 09

Who is your doctor at Natural Health & Wellness? Any comments about him?  Dr. Allred. He’s great. Easy to talk to & very understanding.

Describe your results and/or changes that have taken place since you began care until the present.  I am able to sleep all night now without my arm waking me up. My hips are not sore and my neck is explainable.

Any additional comments about the advance techniques used in the office, the staff or anything else you’d like to say that my help others in their journey to better health.   Appreciate the whole body effort.
