What problems were you having that caused you to seek treatment at our office? I would get very dizzy when I turned by head.

Your problems are a result of what?   The dizziness just came on for no reason other than turning my head.

How long have you had these problems?  I remember having these  problems in the early part of 1994. It lasted about 2 weeks.

Please state how these health problems have interfered with your normal life’s activities and relationships?  I felt very limited in what I was able to do.  

How did you find out about our office?   We received a flyer in the mail.

Who is your doctor at Natural Health & Wellness? Any comments about him?  Dr. Sanders. We think he is very caring, compassionate, pleasant and helpful.

Describe your results and/or changes that have taken place since you began care until the present. I do believe that Dr. Sanders has corrected my problems. I feel like my old self again.

Any additional comments about the advance techniques used in the office, the staff or anything else you’d like to say that my help others in their journey to better health.   We like his “Hi- Low” table. It makes it a lot easier on us old folks. We really like the activator, it’s a cool tool. Your staff is very friendly and nice.
